You made a bold statement that EVERYONE in the Bible was baptized in the Name of Jesus. Then you backpeddle like a politician when called out.
I mentioned two cases where it doesn't mention what was said over the person. Since you were not there, and no documentation was preserved, at best you can make a somewhat educated assumption.
I know how to get you out of the little puddle you created with the broad assumptions. But you are the self perceived genius on everything holy and sacred. You'll figure it out.

Epley makes a reasonable conclusion here based upon what evidence we have, IMHO. But without the specificity that you are requiring I think we should be gracious toward those sincere Christians who differ on this subject.
That is the next reasonable conclusion I'm looking for Bro. Epley to make.

The PATTERN is clear the command Mt. 28:19, Luke 24:47
the practice Acts 2:38, 8;12,8:16, 10:48, 19:5, 22:16
The inferences in the Epistles Rom. 6;3, Gal. 3:27, 1Cor 6:11.
The Apostles were NOT like false teachers today who did one thing in one city and another way in another city.
There are NO Christians who have not been baptized in Jesus Name!

Revend Eeply, would you concede that there are some Christians who may not say the EXACT thing we do at baptism?
Granted one must call on the name of Jesus for salvation.

... what about being filled with the Holy Ghost as evidenced by speaking in other tongues? Are there (or were there ever) any Christians who did not experience this?

I am not Br. Eeply but as he so aptly says. Not one person will be in heaven that has not obeyed Acts 2:38.

Can you (or Bro. Epley) identify a single individual who will be in heaven who lived between the years 200 AD and 1913 AD?
One person, baptized in Jesus' name and filled with the Holy Ghost as evidenced by speaking in other tongues?

Aren't we a wee bit niave (or is it arrogant) if we don't acknowledge there are multiple ways of interpreting Acts 2:38?

No and I can't say if you will be there either. What I can say is if ANY person doesn't obey Acts 2:38 the will not be in heaven.

You can interpret it any way you wish but if you don't do what it says you won't make it.

Aren't you a bit concerned that there was not a single person "who obeyed Acts 2:38," according to your interpretation of the verse, for around 1,700 years of the church's 2,000 years of existence?
Not one. Nobody?
