Written creed or statement of faith? Attach copies
Form of worship? Describe
Formal code of doctrine and discipline? Explain
Distinct religious history? Explain
Literature? Describe
Hierarchy or ecclesiastical government? Describe
Schedule of regular service? Describe nature of services, provide representative literature (brochures, church bulletins, etc.)
Attendance numbers?
Established place of worship?
Property ownership?
Established membership?
Number of members?
Process of becoming a member? Describe
Voting rights of members? Describe
Permission for members to be associated with other churches or denominations?
Are members family?
Conduct of baptism, wedding, funerals?
Minister or leader? Describe role and function, including license-ship
Do you ordain, license, or commission? Explain process
Are you a part of a group of churches with similar beliefs? Explain
Church charters? Provide copy
Any additional info to share?
What church or pastor in their RIGHT MIND would ever give that kind of information to the IRS? You're asking for the devil to come and find you and take you out.
And for what? A tax status that all churches automatically have by rights? To save money on building or property taxes, including a parsonage, or for a salary?
