I wouldn't sign it; big surprise
I know of many churches that currently have this type of requirement. My main issue is that it boxes off/out some saints from participating.
I know of an individual that is quite talented on piano/organ. In the past did play for services but as more and more younger ones became competent to play was no longer used.
Later they instituted the platform standards. This person follows the letter of the law....hair,clothing, jewelry, etc The guilt alone would kill them!
This person was not comfortable signing this type of "contract" and therefore did Not.
Sunday service on holiday weekend.............many many families gone out of town. Pastor announces they will have to sing without music since no one could play......lie, lie, lie....the talented individual was sitting in the pew ; BUT had not signed the paper

I call that just plain dumb!

.....when we lived in another state (1972) they had a much simpler but nonetheless" required statement" to sign to be used in the SS dept. Wish I had a copy...

I was young and stupid, what can I say, I signed it!
