Are we speaking or thinking our way to sickness or spiritual defeat?

For the most part NO we do not "speak things" to be or not to be.
Jesus speaking to the waves or to a tree had nothing to do with his tongue or his positive expression and everything to do with the fact he was God Almighty.
I do believe a positive mental attitude can help one's perspective just as a negative one might foster depression etc., however, this is not the same as speaking things into existence.
The thing we most need is a knowledge of the will of God, and the humility to say in every prayer, "Nevertheless, not my will but thine be done".
