I know this question is to Dan, but practically speaking, yes, it would have almost always had alcohol in it.
Squeezing grape juice out of grapes is a labor intensive process which does not result in a lot of juice for all the work put into it. Furthermore, unless it was drunk within hours after it was squeezed, it would have started to go bad.
Fermenting grape juice into wine is a CONTROLLED process that keeps the juice from going bad. Left to its own devices grape juice would turn into a stringy smelly mess just sitting on the counter, but if allowed to ferment just right, results in wine. Even the "Good Stuff", which is what Jesus produced!
The same can be said for cheese.... which is the controlled "spoilage" of milk.
