The scripture refers to brothers like we do (siblings) and the church family. But perhaps one of the most common references to a brother or brethren is Hebrews (Jews) referring to fellow Jews.

Romans 9

[2] That I have great heaviness and continual sorrow in my heart.
[3] For I could wish that myself were accursed from Christ for my brethren, my kinsmen according to the flesh:

Here Paul laments that his brethren according to the flesh (fellow Jews, who were lost) could be saved.

So his brethren (in the flesh) could become his brethren (in the church).

Another example of the Israelites referred to as brethren;
Deuteronomy 18

[15] The LORD thy God will raise up unto thee a Prophet from the midst of thee, of thy brethren, like unto me; unto him ye shall hearken;

I think this is a good example of Paul being a Jewish brother (in the flesh) who was about to receive his sight and receive the Holy Ghost (becoming, but not yet being, a spiritual brother).
